How to make a medication list that helps

Gather every bottle, box and blister pack in one place, including the ones in the bathroom cabinet and handbag. Copy each label rather than working from memory. A list that's written from the labels is far more useful to a pharmacist than one written from recall.

MedlinePlus recommends keeping a list of all medicines, including over-the-counter ones, and reviewing it with your providers and pharmacists. Keep one copy at home and one in a wallet or bag, and bring it to hospital stays.

Put the date you last checked it at the top. An old list can be worse than none, because people trust it.

What should a medication list include?

Every prescription medicine, over-the-counter medicine, vitamin and supplement. For each one: the name, strength, how it's taken according to the label, when, and what it's for. Add allergies and reactions, the prescriber and pharmacy, an emergency contact, and the date you last checked the list.

Should vitamins and over-the-counter medicines be on the list?

Yes. MedlinePlus advises keeping a list of all medicines, including prescription and over-the-counter medicines, and the Health in Aging Foundation also includes vitamins and supplements. They matter when a clinician reviews everything together.

How often should I update my medication list?

Whenever something starts, stops or changes, and before every appointment or hospital visit. Write the date you checked it at the top so everyone knows how current it is.

Is my information uploaded anywhere?

No. Everything you type is saved only in this browser on this device, so you can come back and edit it. Use “Clear all” to erase it. Printing or saving as a PDF happens on your device.

Can this tool tell me the right dose?

No. Copy the instructions exactly as they appear on the prescription label. The tool never suggests, calculates or changes doses. Ask a pharmacist or prescriber about anything that's unclear.
