Keep “not sure” honest

The most useful logs separate “taken” from “I think so”. If nobody is sure whether a dose was taken, write “?” and check the medicine's instructions or ask a pharmacist. Don't give another dose to be safe.

What should a medication log record?

For each scheduled dose: whether it was reported taken, skipped, or nobody is sure, the time, and the initials of whoever recorded it. Keep “not sure” separate from “taken”. An honest “?” is more useful to a clinician than a guessed tick.

Is a medication log proof that a medicine was taken?

No. It's a record of what people reported. It's still valuable for spotting patterns and for conversations with a pharmacist or doctor.
