Why handoffs matter

MedlinePlus lists medication errors among the problems that can happen at any point from prescribing to taking a medicine. In families, the risky moments are often the changeovers, when two people each assume the other gave the evening dose, or nobody is sure.

A written handoff, one named person per task and one shared record reduce that guesswork. When the record is unclear, the safest step is to pause and ask, not to give another dose.

How can family caregivers coordinate medication reminders?

Agree on one written medication list, name one person responsible for each task, and use one shared record of what's been given or reported so nobody works from memory. When records disagree or someone isn't sure, stop and check rather than giving another dose. Ask the pharmacist if it's still unclear.

What should a caregiver handoff include?

The current dated medication list, the schedule, allergies, where medicines are kept, what has already been given today, any uncertain doses, pending refills, the pharmacy contact, open questions for clinicians and emergency contacts.

Does this replace medication reconciliation?

No. Medication reconciliation is a professional review done by a pharmacist or clinician. This checklist helps families hand over information clearly and bring the right questions to that review.
